Synopsis: Undocked toolbar Customize button brings up one customization window per click

Description
For an undocked toolbar, clicking on Toolbar Options button and selecting
Customize causes a customization window to come up.  But selecting
Customize a second time on the same undocked toolbar brings up a second
customization window.  This is not only potentially very confusing
but can lead to a crash.  I don't have the exact sequence for 
reproducing a crash but can probably come up with one if you really need it.
