Synopsis: Move the upload button on the problem report submission screen

Description
I keep submitting problem reports thinking I've attached files, and find that I've forgotten to press the Upload button after browsing to the file. I think part of the problem has to do with the placement of the Upload button. Some alternative implementations:

1) Validate the form, and if there's text in the attachments field but upload wasn't pressed, ask the user to confirm the upload.

2) The Yahoo model (which I find works very well). Only have an "Attach files" button. Clicking it takes the user to a separate page where they can enter multiple file names, then click an Upload button to transfer the files and return to the original message.

3) Put the Upload below the text field, where it will stand out as part of the flow of filling out the form.
